A pioneering HIV cure trial conducted in Durban, South Africa, has yielded promising results, marking a significant milestone in the global fight against HIV/AIDS.

Led by Professor Thumbi Ndung’u, Director of the Sub-Saharan African Network for TB/HIV Research Excellence (SANTHE), the study demonstrated that 20% of participants maintained viral suppression without antiretroviral therapy (ART) for over a year and a half.

The trial explored a novel treatment strategy known as combination immunotherapy, aiming to empower the immune system to control HIV without continuous medication. Participants received ART shortly after infection to suppress the virus, followed by immune-boosting agents designed to enhance the body's natural defenses. Subsequently, under close medical supervision, ART was discontinued to assess the immune system's ability to maintain viral control independently.

Focus on Women in HIV Research.

Notably, the study exclusively involved 20 women, addressing the historical underrepresentation of women in HIV cure research. Given that women are disproportionately affected by HIV, their inclusion is crucial for developing effective, inclusive treatments.

The trial's findings revealed that 30% of participants remained off ART for nearly a year, with 20% sustaining viral suppression for the entire 55-week duration. Remarkably, these individuals continued without medication for an average of 1.5 years post-trial, under ongoing monitoring.

While the treatment did not achieve universal success among participants, the outcomes offer valuable insights into potential pathways for HIV remission. The study was a collaborative endeavor involving the HIV Pathogenesis Programme at the University of KwaZulu-Natal, the Africa Health Research Institute, the Ragon Institute of Mass General, MIT and Harvard, and Gilead Sciences.

The successful execution of this trial in Africa paves the way for future research aimed at achieving a functional cure for HIV, particularly in populations most affected by the epidemic.
